MACMILLAN, MUIR & ASSOCIATES DEFINING THE FIELD OF BRAIN MEDICINE EST. 2026

PRIVATE INTERVENTIONAL PSYCHIATRY

Clinical work, by referral.

We each keep a small clinical practice alongside everything else on this site. It is by referral or invitation, availability is not guaranteed, and we are deliberate about fit. Here is what we do, how people reach us, where we see them, and where we are licensed.

ALL WAYS TO ENGAGE

WHAT WE TREAT

Hard-to-treat conditions, with every tool on the table.

Depression that has not responded to standard care, obsessive-compulsive disorder, PTSD, bipolar depression and anxiety, in adolescents and adults. Both of us are board-certified in adult psychiatry and in child and adolescent psychiatry.

  • Transcranial magnetic stimulation, including accelerated protocolsBoth of us have delivered TMS since 2017 and co-authored the SWIFT trial of accelerated deep TMS.
  • Esketamine and other rapid-acting treatmentsOur former Brooklyn practice was the first in New York City to offer esketamine when it was approved in 2019.
  • Measurement-informed careValidated scales at baseline and over time, and we go well beyond the PHQ-9: passive mood sensing with EmoDTx and structured assessment with Psyrin show us what changes between visits. We advise both companies.
  • Mentalization-based treatment and other psychotherapiesOwen is an official MBT supervisor with the Anna Freud National Centre; Carlene trained in MBT and founded an adolescent DBT program at McLean.

HOW PEOPLE REACH US

From many directions.

Referrals come from treating clinicians, from colleagues in other fields, from family offices, private care navigators and educational consultants, and by word of mouth from people we have treated. If a current or former patient suggested you get in touch, say so when you write.

Send a short note to info@macmillanmuir.com: what you are hoping for, what has been tried, and where you live. We read every message ourselves and usually answer within hours. When we cannot take someone on, we say so and, where we can, point to colleagues or to Radial, the clinical network we co-founded.

Please keep the first note to what is needed to judge fit. Writing to us does not establish a physician-patient relationship. In an emergency, contact local emergency services or call or text 988 in the United States.

WHERE WE SEE PEOPLE

Telehealth to start. Then wherever the work is.

A telehealth visit is often the right way to begin. For interventions, we work from a discreet office setting in Connecticut, where complete privacy, with no other patients on the premises, can be arranged in advance. We also travel for in-home TMS and for consultations.

WHERE WE ARE LICENSED

Licensure, as of November 2026.

Licensure does not mean availability. These are the jurisdictions where each of us holds an active medical license and can lawfully see patients, in person or by telehealth.
